Protecting Mobility for People Living with Diabetes

Activate Me

Activate Me focuses on preventive care for people living with type 1 or type 2 diabetes, whose feet and lower limbs can be affected by the condition over time.

Through careful assessment and complementary therapy, the program is designed to help protect foot and limb health — and the mobility and independence that depend on it.

The Problem

The Problem We Address

Diabetes can quietly affect the nerves and blood flow in the feet and lower limbs. Minor injuries may heal slowly and, without proper care, can become serious — putting a person's mobility and independence at risk.

Nerve & Circulation Changes

Diabetes can affect sensation and blood flow in the lower limbs.

Slow-Healing Injuries

Small foot injuries can heal slowly and worsen without care.

Risk to Independence

Untreated complications can threaten mobility and daily independence.

The Evidence

Why This Matters in San Antonio

Diabetes is a serious national issue — and an especially urgent local one in Bexar County, where Joy of Giving USA is based.

15%

of adults in Bexar County have diagnosed diabetes — about 239,000 people, higher than Texas (14%) and the U.S. (12%).

68.7

diabetes-related amputations per 100,000 people in Bexar County.

Americans — about 1 in 8 — live with diabetes.

Diabetes is the leading cause of non-injury lower-limb amputations in the U.S. — many of which begin with foot problems that preventive care can help address.

People in vulnerable and underserved communities

Our Approach

How the Program Works

We pair careful clinical and vascular assessment with preventive care and complementary therapy to help protect foot and limb health. Technology supports the clinical work — it is not the goal.

The Problem

Diabetes complications in the feet and lower limbs that can threaten mobility.

Assessment

Clinical and vascular evaluation of foot and limb health.

Intervention

Preventive care with complementary electro-stimulation therapy.

Goal

Protecting foot and limb health, mobility and independence.

Assessment + Preventive Care + Technology → Protected Limb Health → Mobility → Independence

How It Works

How Activate Me Works

Every program follows the same general, person-centered path — the clinical detail is tailored to each person by qualified professionals.

1

Referral & Intake

A person is referred or reaches out, and we gather their history and goals to see how the program can help.

2

Assessment

Qualified professionals evaluate each person's condition and needs before any treatment begins.

3

Personalized Plan

A rehabilitation plan is designed around the person's specific situation and objectives.

4

Guided Sessions

The person attends structured therapy sessions, supported and adjusted by their care team.

5

Specialized Technology

Rehabilitation technology is applied as a tool within the plan — never as the goal in itself.

6

Progress & Follow-up

We track progress against each person's goals and adapt the plan as they improve.

Technology & Clinical Approach

Technology & Clinical Approach

The approach below is provided for professional and funder review. Care is always delivered and adapted by qualified professionals.

Preventive & clinical approach

Activate Me is designed to support people living with diabetes and to help protect foot and limb health through early evaluation and preventive care.

The program pairs clinical and vascular assessment of the feet and lower limbs with complementary electro-stimulation therapy, used as a supportive tool alongside standard medical care.

What the program includes

  • Medical consultation and referral related to diabetes management.
  • Clinical evaluation of vascular status of the lower limbs (for example, ankle-brachial index assessment).
  • Evaluation for signs of infection or tissue damage, with imaging when appropriate.
  • Complementary electro-stimulation therapy, applied under professional supervision.
  • Classification of each participant's risk so the plan can be adapted to their level of need.
Program Goals

What Success Looks Like

Because Joy of Giving USA is building this program, we describe the outcomes it is designed to support — not results we claim to have already achieved in the U.S.

  • Earlier identification of foot and limb risk factors
  • Better adherence to preventive care
  • Reduced risk of preventable complications
  • Maintained mobility and independence
  • Timely referral to additional medical care when needed

What We Measure

These are the indicators Joy of Giving USA is designed to track, so funders can understand how a grant would be evaluated.

  • Participant enrollment
  • Risk factors identified
  • Preventive interventions completed
  • Adherence
  • Functional mobility
  • Referrals to additional medical care when required
  • Program completion
For Foundations & Partners

What Foundation Support Makes Possible

Activate Me

Expand access to preventive foot and limb care for people living with diabetes.

Foundation support can help:

  • Expand access to preventive care for people who cannot otherwise afford it
  • Provide clinical and vascular assessment delivered by qualified staff
  • Increase access to specialized technology used in preventive care
  • Subsidize care for individuals facing financial barriers
  • Increase program capacity so more people can be served
  • Support outcome measurement
